๐ What is Edema?
Edema is an abnormal accumulation of fluid in the interstitial tissue causing visible swelling.
It can be generalised (whole body) or localised (limited to one part) depending on the underlying cause.
๐ฉบ Causes of Generalised Edema (เคธाเคฎाเคจ्เคฏ เคธूเคเคจ เคे เคाเคฐเคฃ):
-
Heart Failure (เคนृเคฆเคฏ เคी เคเคฎเคोเคฐी) – Reduced cardiac output leads to fluid retention.
-
Kidney Disease (เคिเคกเคจी เคी เคฌीเคฎाเคฐी) – Protein loss or sodium retention causes puffiness around eyes and legs.
-
Liver Cirrhosis (เคฒीเคตเคฐ เคธिเคฐोเคธिเคธ) – Decreased albumin synthesis causes fluid accumulation in abdomen (ascites).
-
Protein Deficiency (เคช्เคฐोเคीเคจ เคी เคเคฎी) – Seen in malnutrition or intestinal loss of protein.
-
Endocrine Disorders (เคนाเคฐ्เคฎोเคจ เคी เคเคก़เคฌเคก़ी) – Hypothyroidism can cause facial puffiness.
-
Drugs (เคฆเคตाเคเคฏों เคे เคฆुเคท्เคช्เคฐเคญाเคต) – Steroids, NSAIDs, calcium channel blockers can cause edema.
๐ Causes of Localised Edema (เคธ्เคฅाเคจीเคฏ เคธूเคเคจ เคे เคाเคฐเคฃ):
-
Infection or Inflammation (เคธंเค्เคฐเคฎเคฃ เคฏा เคธूเคเคจ) – Cellulitis, abscess.
-
Venous Obstruction (เคถिเคฐाเคं เคฎें เคฐुเคाเคตเค) – Deep vein thrombosis (DVT).
-
Lymphatic Obstruction (เคฒเคธीเคा เคฎाเคฐ्เค เคฐुเคाเคตเค) – Filariasis, surgery, or tumor compression.
-
Allergic Reaction (เคเคฒเคฐ्เคी เคी เคช्เคฐเคคिเค्เคฐिเคฏा) – Angioedema, insect bite.
-
Trauma (เคोเค) – Local tissue injury.
๐ฌ Investigations for Edema (เคธूเคเคจ เคी เคांเค):
๐งช Blood Tests:
-
CBC, ESR
-
Kidney function test (Urea, Creatinine)
-
Liver function test (Albumin, Bilirubin)
-
Thyroid function test (TSH, T3, T4)
-
Serum electrolytes (Na⁺, K⁺)
๐ง Urine Tests:
-
Urine protein, routine and microscopy
-
24-hour urine protein (for nephrotic syndrome)
๐ฉป Imaging Studies:
-
Chest X-ray – for heart failure
-
Ultrasound Abdomen – for liver, kidney, ascites
-
Doppler study – for DVT or venous obstruction
๐ฉน Conclusion (เคจिเคท्เคเคฐ्เคท):
Edema is a sign, not a disease. Identifying the underlying cause through proper investigations is essential for effective management.
